按照智商分数和社会适应障碍程度两个基本指标将儿童智力障碍分为四个等级。

①轻度智力障碍

智商70~55分(以韦氏智力测验为例,下同),同时具有轻度的社会适应障碍。轻度智力障碍儿童具有基本的生活自理能力,能承担简单的家务劳动。早期教育对他们的发展十分重要,他们可以进入幼儿园、学前班接受早期康复训练和学前特殊教育,在小学正常班级随班就读。他们在学科学习(如语文或数学等课程)方面存在不同程度的困难,但大多数人能完成小学阶段的基本学习,具有一般日常生活所需的语言交往能力.具有简单的阅读和应用写作能力(如写留言条、请假条、简单的应用文书等),具有简单的计算能力;青少年期可以接受职业教育和职业训练,成年后可以从事简单的职业劳动,参与社区生活。他们在生活环境变化时需要得到他人的支持和帮助。

②中度智力障碍

智商55~40分,同时具有中度的社会适应障碍。中度智力障碍儿童具有一定程度的生活自理能力,可以完成部分简单的家务劳动,在基本生活方面需要得到他人的支持和帮助。他们一般较早被发现,并在3岁前后被确诊,可以及时进行早期康复训练,在康复站或学前特殊教育班接受学前教育,进入义务教育阶段可以在特殊教育学校或班级接受以适应日常生活为主的功能性教育,能够认识常见的文字和进行简单的计算。少数中度智力障碍儿童具有一定的特殊能力,如音乐能力,在专门训练下可以达到一定的水平。部分中度智力障碍儿童可能存在语言方面的沟通问题或行为问题,可以借助于康复训练和行为支持得到显著改善。经过一定的职业训练,在适当的支持下,他们可以在专门的工作岗位或在正常的工作环境下从事生产劳动,参与社区活动,与周围人建立友谊和信任的关系。

③重度智力障碍

智商40~25分,同时具有严重的社会适应障碍。重度智力障碍儿童常在出生后不久被确诊,应当对其积极进行早期干预与训练。对他们的训练主要集中在生活自理、简单语言沟通和人际交往方面。经过长期系统的训练,重度智力障碍儿童的生活功能会得到显著的改善,独立生活能力得到增强;也可以为他们安排休闲活动或力所能及的劳动,充实他们的日常生活。为改善他们的日常社会生活,可以依据重度智力障碍儿童生活的具体环境,评估他们在特定环境中的生活状}兑,为他们制定在这一特定生活环境中的训练目标和训练计划,并适当地改变环境,使用辅助工具,帮助他们实现基本的生活功能,参与社区生活,使他们生活得愉快和幸福。

④极重度智力障碍

智商在25分以下,同时存在极为严重的社会适应障碍。极重度智力障碍儿童基本没有独立的生活能力,常常伴随着多重障碍,包括运动障碍、日常生活障碍、言语沟通障碍和心理方面的疾病等,需要得到长期持续的支持和帮助,但他们也具有一定的潜能。他们可以接受一定的康复训练和必要的治疗,在一定程度上改善生活自理状态,表达需求和情感等。可以为极重度智力障碍儿童提供各种行为支持和身体协助,让其感觉到周围人对他们的关心、尊重,让他们愉快地生活。

目前国际上对智力障碍采取了一种新的、根据支持程度进行的分类,将智力障碍分为需要间歇性支持、有限支持、广泛支持和全面支持四类。新的分类方法为智力障碍儿童的康复提供了新思路,可以通过建立支持系统为智力障碍儿童的康复带来新的途径和方式,能够有效地改善康复训练和康复服务的效果,最终目的在于提高他们的生活质量。

韦氏智力测试表

韦氏智力量表(wechsle。Intelligence Scale)由美国心理学家韦克斯勒所编制,是继比内一西蒙智力量表之后为国际通用的另一套智力量表。 韦克斯勒长期从事心理测验的编制和研究工作,为发展国际知名的智力量表做出了极大的贡献。l939年他首先编制成韦克斯勒一贝勒维量表(w—B),司用于成人及儿童。随后又编制出平行本,称w—BⅡ,因此称前孝为w—B l。1949年将W—BⅡ发展和修改成韦氏儿童智力量表(Wechsler

Intelligence Scalefor Children,WISC).成为继比内测验之后又一个应用最广的儿童智力量表。1955年将w—B I修订成韦氏成人智力量表

(wechsler AduItIntelligence Scale,WAIS),使之与WISC相衔棒.l967年又编制了韦氏学龄前及幼儿智力量表(Wechsler Preschool and Primary scale:f。;nteuigence,wPPSi),至此一套从4岁幼儿到成人(74岁)的三个着名智力量表编制成功。。70年代初,韦氏着手修订他自己编制的智力量表,1974年出版了韦氏儿童智力量表修订本(WISC—R)。1981年出版了韦氏成人智力量表修订本(wA:s_IR2 1989年出版了韦氏学龄前及幼儿智力量表修订本(WPPSl—R)。此外,l991年出版’r韦氏儿童智力量表第三版(WISC一Ⅲ),1997年出版了韦氏成人智力量表第三版

(WAIS一Ⅲ)。 韦氏智力量表主要指WAIS—R、WISC—R和WPPSl这三个量表,三者均包括相同的分测验,因年龄关系,有一些在形式上作了一些变更,还有少数量表中的分测验有增减(见表3—4)。注:“[ ].‘内项目为在WISC—R和WPPSl中的备用分测验1979~1981年问,在龚耀先主持下完成基于WAIS的修订工作,称中国修订韦氏成人智力量表(WAIS—RC)。考虑到中国城市和农村的现实情况,WAIS—RC分别制定了城市和农村两个版本。城市和农村两式的测验项目相同,记分标准也一样,但各分测验项

目的难易排列顺序和计算量表分与智商的标准不同。 两式各包括11个分测验,其中言语部分包括知识、领悟、算术、相似性、数字广度、词汇6个分测验,操作部分包括数字符号、图画填充、木块图、图片排列、物体拼凑5个分测验。

1.知识

韦克斯勒认为,智商越高的人,兴趣越广泛,好奇心越强,所以获得的知识就越多。故此测验主要测量人的知识广度、一般的学习及接受能力、对材料的记忆及对日常事物的认识能力。

2.领悟

此测验主要测量判断能力、运用实际知识解决新问题的能力以及一般知识。该测验对智力的G因素负荷较大,与知识测验相比,受文化教育影响小,但记分难以掌握。

3.算术

此测验主要测量数学计算的推理能力及主动注意的能力。该能力随年龄而发展,故能考察智力的发展,同时对预测一个人未来心智能力很有价值。

4.相似性

此测验设计用来测量逻辑思维能力、抽象思维能力与概括能力,是G因素的很好测量指标。

5.数字广度

此测验主要测量人的注意力和短时记忆能力。临床研究表明,数字广度测验对智力较低者测的是短时记忆能力,但对智力较高者实际测量的是注意力,且得分未必会高。

6.词汇

本测验主要测量人的言语理解能力,与抽象概括能力有关,同时能在一定程度上了解其知识范围和文化背景。研究表明,它是测量智力G因素

的最佳指标,可靠性很高。但其记分较麻烦,评分标准难掌握,实施时间也较长。

7.数字符号

该测验主要测量一般的学习能力、知觉辨别能力及灵活性,以及动机强度等。该测验与工种、性别、性格和个人缺陷有关,不能很好地测量智力的G因素,但具有记分快、不受文化影响的特点。

8.图画填充

此测验主要测量人的视觉辨认能力,以及视觉记忆与视觉理解能力。填图测验有趣味性,能测量智力的G因素,但它易受个人经验、性别、生长环境的影响。

9.木块图

该测验主要测量辨认空间关系的能力、视觉结构的分析和综合能力,以及视觉一运动协调能力等。在临床上,该测验对于诊断知觉障碍、注意障碍、老年衰退具有很高的效度。

10.图片排列

此测验主要测量被试者的分析综合能力、观察因果关系的能力、社会计划性、预期力和幽默感等。它也可以测量智力的G因素,可作为跨文化的测验。但此测验易受视觉敏锐性的影响。

11.图形拼凑

此测验主要测量处理局部与整体关系的能力、概括思维能力、知觉组织能力以及辨别能力。在临床上,此测验可了解被试的知觉类型,他对尝试错误方法所依赖的程度,以及对错误反应的应对方法。此测验与其他分测验相关较低,并对被试的鉴别力不甚高。

编辑本段韦氏智力测试表使用注意事项:

(一)在操作修订韦氏量表时,一定要按本量表的标准程序进行。这些程序在手册中均有规定,所以采用此量表的人员,一定要阅读手册。除非在临床应用时,因某些特殊情况,在不得已的情况下可进行适当变动。

(二)主试者必须受过进行个别和团体测验的训练,掌握本量表的测量技术——提问技术。鼓励回答的技巧、书写回答格式、记分方法、记分标

准、原始分(粗分)换算标准分(量表分)的方法,计算智商的方法、对结果作解释等。

(三)测验材料有组织,以方便测验时取用,能得心应手,不致紊乱,不影响进行时间。主试者井井有条,被试者操作自由;主试者忙乱不堪,对被试的操作带来不良影响。

(四)测验时间要选择恰当,这是与被试者建立良好协调关系所必需的。被试者应在精力充沛、身体舒适、没有急事的时候来接受测验。

(五)主试者应努力取得被试者的合作,尽量使他们保持对测验的兴趣,用如下一些鼓励语言往往是有效的。如:“好”,“这不花你许多时间吧”。

(六)测验的程序和时间

(1)测验的程序:10个正规测验交叉进行,使整个测验过程更加有趣并富于变化,调动儿童应试的积极性,防止儿童疲劳和厌烦.

(2)测验所需时间:10个测验进行完毕大约需要1―1.5小时,如真有困难也可分两次来进行,间隔时间不要超过7天.

(七)测验的场所

(1)测试室:室内要安静,避免噪声和其他干扰;要有良好的照明和合适的桌椅.

(2)人员设置:主试和被试.

韦氏智力测试表测验实施的一些具体要求:

(1)一般正常三年级以上的儿童与65岁以下成人均可团体施测,幼儿、智力低下者和不能自行书写的老年人则可个别施测。

(2)测验开始时先发给记录纸,要求填好姓名、性别、年龄等项。然后发下测验图册,请被试者打开第一页(A1)说:“看上面一张图,图下角缺一块,请你从下列的6块图片中选一块最合适的补上去。你找找看,哪一块补上去最好”。先让被试者尝试一下,最后将正确答案“4”号告诉大家,并请他们将数码“4”写在记录纸上与A.对应的空格里。个别施测者,由被试者指点出他确认的图形,由主试在记录纸上记录相应的号码。

(3)在被试都掌握了方法后,接着翻到下一页(A2),并告诉大家以下每图都有缺少的一角,要求从下面的几个小块图中找到一个最合适的补上去,并把它的号码写到记录纸相应的空格内。“注意,A:的答案应是‘5’。大家对一下有无选错、写错,错了可以改正”。稍停,“好,现在开始一页一页做下去。注意不要翻过了页,不要跳过去做,要对好题号写,不要写错位置”。

(4)“本测验限在40分钟内交卷,能做多少做多少。”说完,开始记时。测验进行到20分钟及30分钟时各报一次时间,请大家在刚完成的答案下划一记号“——”。测验时间满40分钟时,不管是否做完,一律交卷。

(5)幼儿及弱智者在个别施测中,当进行到c、D、E三单元时,每单元如连续3题不通过,则该单元不再往下测,未测项目都按不通过计,但A、AB、B三单元不管做对多少都必须做完。

(6) 掌握好测试的起点和停测点.

8―10岁:8岁零月零天―10岁11月30天

6―7岁:6岁零月零天―7岁11月30天

本测验题一律为二级评分,即答对给1分,答错为0分。被试在这个测验上的总得分就是他通过的题数,即测验的原始分数。

本测验的量表分数是先将被试的原始分数换算为相应的百分等级,再将百分等级转化为IQ分数。例如一个16岁城市儿童测得原始总分为55分,先查百分等级常模表得55分相应的百分等级为70,再查智商常模表得70百分等级的IQ为108。
