2012年11月9日放学回家路途中,因甲方将鞭炮点燃投掷于乙方背后,乙方惊吓逃跑,致使乙方摔到,11月10日经宜宾骨科医院诊断桡骨远端骨折，现经双方协商一致，自愿达成如下协议，以便共同遵守。

一、2012年11月9日至10日到新市、宜宾骨科医院检查过程中产生的一切费用由甲方承担。

二.甲方另赔偿乙方后期治疗费2500元，甲方在2012年11月15日之前将后期治疗费交到乙方，乙方收到甲方给付赔偿后，不再以任何理由向甲方提出任何要求。

三.本协议一式四(肆)份。自甲、乙双方签字(盖章)起生效。
