1 资料与方法

1.1 一般资料

本次研究选择了于2009年6月至2011年6月到我院进行治疗的肱骨髁上骨折患儿共40例, 其中包括男18例, 女22例;其年龄在5~10岁之间, 平均年龄为 (6.3±1.5) 岁。患者中有26例为, 14例为右侧;有28例患儿为伸直型, 11例为屈曲型13例为桡偏型。在受伤后患者进行就诊的时间为0.5~5d, 平均时间为 (2.5±1.2) d。受伤的原因都是跌伤, 而且都是闭合性的骨折, 没有出现血管神经损伤等并发症。

1.2 手术方法

在进行手术之前对所有的患儿进行麻醉, 方法是基础的麻醉加上臂丛神经阻滞麻醉。之后患儿采取仰卧位, 相关的护理人员协助其将患肢肘关节屈曲并放在胸前。在患儿的肘关节外侧, 沿着肱骨外髁向上方进行纵向的皮肤切口。切口的长度保持在3cm左右, 沿着肱三头肌和肱桡肌的间隙进入到骨折断端, 之后沿着肱骨的前后对骨膜进行剥离, 将骨折断端的血肿以及软组织部位进行仔细的清理, 在直视的状态下将患儿的折骨进行复位。选择直径为1.5~2.0mm的克氏针从患儿的外髁部位以40°~50°的角度斜穿骨折线到内上方, 穿透内侧的骨皮质进行固定。之后将患儿的肘关节进行轻微伸直, 在对其肱骨内髁骨性标志清楚之后, 使用大拇指触及其尺神经同时推至后方。之后使用克氏针从患儿肱骨的近

端外侧以40°~50°的角度进行斜穿骨折线到内髁骨性标志后固定, 这时不需要一定穿透其内侧骨皮质。然后在两克氏针的钉尾间使用钢丝作张力带加以固定。肘关节进行简单的伸屈活动, 检查钢针是否固定牢固, 同时对患儿的肘关节内外翻情况进行检查, 必要时要在C臂X机透视肘关节进行正侧位。最后将钢针的尾端弯曲后埋在皮下, 在切口内放置引流管。对伤口进行逐层缝合。将肘部弯曲90°之后, 取掌心向上位使用石膏托进行固定。在术后3周去去掉石膏进行适当的功能性锻炼, 根据患儿的恢复情况在4~6周拔除克氏针, 同时对肘关节的功能锻炼要相应的加大。另外患儿还需要在术后的第l周、第1.5个月及第3个月进行复查, 拍摄肘关节的X线片, 了解骨折的位置以及愈合情况。

2 结果

本次研究的40例患儿中有37例获得随访, 平均时间为3.2年。所有的患者在手术后的患肢均恢复良好。在手术后的3周进行复查X线片, 显示骨痂生长情况良好, 骨折线模糊。在手术后的第4~6周进行复查时拍摄X线片, 显示骨折线已经完全消失, 内固定克氏针没有松动断裂的迹象。手术后的第4周复查X线片显示:骨折完全愈合, 拔除克氏针。没有缺血挛缩情况发生。

3 讨论

3.1 据有关的统计显示, 肱骨髁上骨折在占儿童的全身骨折中占到了3%~26%, 是肘部最为常见的。

此病的发病率如此高主要是由于肱骨髁上部处于骨密质以及骨松质的交界部分, 前面有冠状窝, 后面有鹰嘴窝, 而且中间只是有一层很薄的松质骨片, 相对较薄弱;另外又因为这个部位是受力的薄弱点;也因为儿童的结构的特点肘部相对坚固, 在跌倒时就很容易造成肱骨髁上骨折。

3.2 在小儿肱骨髁上骨折手术入路的选择方面。

主要有传统的肘后正中切口、肘前内侧切口以及肘外侧切口和肘内侧切口等。肘后正中切口是进行“v”形的切开, 在直视的情况下做内固定, 但这种方法虽视野清晰, 但是却破坏了肱三头肌的完整性, 肘关节的功能产生不利的影响, 目前来说, 在临床上以较少使用此法。本次研究的患儿治疗均采用外侧切口入路的方法, 不但对软组织的损伤小而且也不容易导致缺血性肌挛缩情况出现。而且此种方法又可以将手术视野充分的暴露, 露出骨折端外侧和前后侧的皮质, 能够将侧方移位以及选择移位很好的复位, 在直视的情况下进行复位, 可以更好的确保效果, 同时也可以在很大的程度上减少并发症的发生。

总之, 使用外侧小切口入路克氏针加张力带内固定对儿童肱骨髁上骨折进行治疗, 具有创伤小、内固定可靠的优点, 而且并发症较少、功能恢复快而且好, 能够有效的预防内翻等并发症, 值得在临床上推广使用。

The lady went home with her mousetrap, but when she looked in her cupboard, she could not find any cheese in it. She did not want to go back to the shop, because it was very late, so she cut a picture of some cheese out of a magazine and put that in the trap.

Surprisingly, the picture of the cheese was quite successful! When the lady came down to the kitchen the next morning she found a picture of a mouse in the trap beside the picture of the cheese!

有一天某位女士看到一只老鼠在自家的厨房地板上窜过。她很害怕老鼠,所以她冲出屋子,搭上了公共汽车直奔商店。在那儿,她买了一只老鼠夹。店主告诉她:“放点奶酪在里面,很快你就会逮住那只老鼠的。”

这位女士带着鼠夹回到家里,但她没有在碗橱里找到奶酪。她不想再回到商店里去,因为已经很晚了。于是,她就从一份杂志中剪下一幅奶酪的图片放进了夹子。

令人称奇的是,这画有奶酪的图片竟然奏效了!第二天早上,这位女士下楼到厨房时,发现鼠夹里奶酪图片旁有一张画有老鼠的图片!

故事十五:

Gardening Gloves

For months I hinted that I needed a new wedding ring, since I had developed an allergy to gold. On my birthday, while I was gardening, my husband asked me for gift suggestions. I held my hands up and said, ”Well, you‘ll notice that my hands are bare.“

Later that evening I opened my present with enthusiasm. ”Happy birthday," he said, as I unwrapped a new pair of gardening gloves.
